Find-A-Pet Form

Our Find-A-Pet form can be used in two ways. If you see a pet on our website that you're interested in, list that pet's name and ID number on the form and an Adoption Counselor will call you to discuss. If you don't see a pet on our website that you're interested in, describe the kind of pet you're looking for, and if we get a pet in that matches your request we will notify you.

Please note that Adopt-A-Pet operates in the Chicago and suburban area and only adopts animals to families in the greater Chicagoland vicinity or, on certain occasions, immediately surrounding areas such as southern Wisconsin or northern Indiana.
